Five years after surviving a massacre at a nursing home, Ambar (23), Dika (15), and Rina (28) are living together, trying to cope with their trauma. Their fragile peace is disrupted when Dika is in an accident, requiring Ambar to be hospitalized where Rina works as a nurse. There, they reconnect with Yudi (35), a police officer who also survived the attack, and Priyo (33), another officer hospitalized for a case investigation. Ambar begins to sense the presence of Ivanna, the headless ghost responsible for the nursing home massacre. When a nurse dies without a head, Ambar believes Ivanna has returned and is targeting people at the hospital, though she doesn't understand why. Ambar, Rina, Dika, Yudi, Priyo, and others must now fight to survive and stop Ivanna’s escalating terror.
