Synopsis

Independent Air Flight 1851, traveling from Italy to the Dominican Republic, was scheduled to refuel at an island in Portugal. Moments before reaching the airport, however, the Boeing 707 plunged into a mountain. The international investigation team found no mechanical issues and were baffled by the accident's cause. It was only the cockpit voice recorder that revealed a series of errors, a confluence of mistakes that ultimately led to the tragedy for the 144 passengers aboard.
