Synopsis

On September 7, 2011, a passenger plane carrying the Lokomotiv Yaroslavl hockey team crashed near Yaroslavl, Russia, moments after liftoff. Of the forty-five people aboard, only one survived. The subsequent investigation determined that the pilots lacked adequate training specific to the aircraft model and made errors in handling the plane during the critical takeoff phase.
