Gilbert finds himself burdened by familial responsibilities: his mother, Darlene Cates, struggles with morbid obesity and self-consciousness, while his younger brother, Arnie, portrayed by Leonardo DiCaprio, faces mental health challenges. Gilbert takes on the role of caretaker, working at a failing grocery store where he makes deliveries to the Carver household. There, he shares some playful moments with Betty, Mary Steenburgen’s bored housewife, during her husband's insurance sales trips. This pattern continues without real progress until Becky, played by Juliette Lewis, arrives and sparks a sense of possibility for Gilbert, encouraging him to consider his own needs. A foreseeable tragedy then unfolds, forcing the family to confront difficult decisions. DiCaprio delivers an impressive performance as the teenager on the verge of adulthood; his repetitive speech and childlike outbursts are just part of what makes Arnie so compelling—a portrayal that evokes both fondness and a desire to shake some sense into him. Depp provides stability to the narrative, injecting humor to temper any potential for excessive sentimentality.
