The standard explanation found in most psychiatry textbooks is rooted in Freudian theory; the idea being that a young person, like many growing up, experiences strong sexual attraction toward their mother—the so-called Oedipus complex. However, a head injury can trigger these urges to surface, leading someone to believe they are attracted to their mother and question her identity: “My God, if this is my mother, why am I attracted to her? How come I'm aroused? This must be some other strange woman.” This explanation seems clever but falls short when considering a patient who experiences the delusion regarding his pet dog. He’ll insist it’s not Fifi, even though it looks identical, claiming it has been replaced by another dog. How does Freud account for this unless you start suggesting inherent bestiality in all humans? So what truly causes the Capgras Delusion? It turns out that when we look at an object, the message goes to the temporal lobes—to the visual centers there. But seeing is a multi-step process; after recognition, emotional response is also needed. This is evident with paintings by Picasso or Rembrandt, or even looking at your mother’s face – appropriate warmth must be evoked. Similarly, when viewing a lion, fear should arise. All of this is part of the visual process, but it happens in a different area of the brain. I've proposed that in these patients, the message reaches the temporal lobe cortex, allowing them to recognize their mother and evoke related memories.
